Community Resource & Linkage Coordinator - QMHP
About The Position
The Community Resource & Linkage Coordinator is responsible for assisting individuals and families accessing The Landing, including individuals experiencing homelessness or housing instability, with identifying behavioral health, crisis, social, housing, and community needs and connecting them to appropriate resources and supports. This position works directly with individuals experiencing behavioral health needs, crisis situations, homelessness, housing instability, substance use concerns, financial hardship, and other barriers that may impact safety, recovery, and community stability. The coordinator provides behavioral health support, crisis intervention and de-escalation within the scope of the position, resource navigation, referrals, warm handoffs, care coordination, linkage, and follow-up. The position assists individuals experiencing homelessness or at risk of homelessness with navigating shelters, transitional and permanent housing resources, benefits, identification documents, food assistance, transportation, healthcare, behavioral health and substance use services, employment, and other supports needed to promote stabilization and long-term independence. The coordinator maintains current knowledge of resources throughout Kaufman County and surrounding communities, including emergency shelters, homelessness response programs, housing and rental assistance, crisis services, suicide prevention resources, behavioral health and substance use treatment, hospitals, food assistance, employment and workforce services, transportation, healthcare, benefits, financial assistance, education, veteran services, and other community supports. This position may provide behavioral health screening, needs identification, crisis assessment and support within the scope of agency policy, safety planning support, service coordination, and linkage to appropriate clinical, crisis, housing, and community-based services. The Coordinator recognizes when an individual requires a higher level of care and coordinates with Lakes crisis services, clinical staff, emergency services, shelters, housing providers, and community partners as appropriate. The coordinator works collaboratively with Peer Support Specialists, QMHPs, licensed clinicians, crisis staff, case managers, hospitals, shelters and housing providers, homeless service organizations, law enforcement and first responders, community organizations, and other Lakes programs to support continuity of care and successful community stabilization. Responsibilities include timely documentation, tracking referrals and linkages, participating in multidisciplinary coordination, maintaining service records, and meeting applicable agency, funding, and program requirements. The position develops and maintains relationships with community organizations, service providers, employers, nonprofits, faith-based organizations, governmental agencies, shelters, housing providers, homelessness response organizations, hospitals, schools, veteran organizations, law enforcement, first responders, and other partners. The position represents The Landing and Lakes Regional Community Center at community outreach activities, resource fairs, collaborative meetings, presentations, and other activities designed to strengthen the community resource, homelessness response, and crisis-response network. The Community Resource & Linkage Coordinator assists with identifying gaps in available housing, homelessness, community, and crisis resources; developing new partnerships and referral pathways; and maintaining an organized and current resource network for The Landing staff and participants. Strong communication, engagement, organizational, problem-solving, and relationship-building skills are essential. This position serves as a bridge between individuals, crisis supports, behavioral health treatment, homelessness and housing resources, and other community services, with an emphasis on safety, stabilization, recovery, housing stability, independence, and successful connection to ongoing supports. The successful candidate must meet all qualifications required by Lakes Regional Community Center for designation as a Qualified Mental Health Professional (QMHP) and maintain all required training and credentials in accordance with agency policy and applicable requirements.
Requirements
- Bachelor's degree from an accredited college or university in Social Work, Psychology, Sociology, Human Services, Counseling, Rehabilitation, or another qualifying behavioral health/human services field consistent with QMHP requirements.
- Experience in behavioral health, crisis services, homelessness services, housing, community services, resource navigation, care coordination, outreach, workforce development, or a related setting preferred.
- Strong communication, organizational, engagement, and problem-solving skills.
- Valid Texas driver's license, acceptable driving record, reliable transportation, and personal automobile liability insurance as required by the State of Texas.
- Working computer skills, including Microsoft Office.
Nice To Haves
- Master's degree in Social Work, Psychology, Counseling, Sociology, Human Services, Rehabilitation, or a related behavioral health/human services field preferred.
- Experience providing behavioral health or crisis services as a QMHP.
- Experience working with individuals experiencing homelessness, housing instability, or at risk of homelessness.
- Experience with homeless services, housing navigation, shelters, rapid rehousing, benefits navigation, or other stabilization resources.
- Experience with crisis intervention, de-escalation, safety planning, community resource navigation, care coordination, or service linkage.
- Knowledge of behavioral health, substance use, crisis, suicide prevention, homelessness, housing, and community service systems.
- Knowledge of community resources and service providers within Kaufman County and surrounding communities.
- Experience developing community partnerships and referral networks.
- Experience working collaboratively with shelters, housing providers, hospitals, law enforcement, first responders, behavioral health providers, and multidisciplinary teams.
