About The Position

The Community Resource Coordinator is responsible for coordinating discharge planning, reentry, and reintegration services for individuals releasing from the facility. This position works in collaboration with a multidisciplinary treatment team to identify needs, develop transition plans, coordinate referrals, and support continuity of care upon release. The position assists individuals with linkage to community-based services, behavioral health and substance use supports, MAT-related discharge planning, housing resources, identification documents, benefits, transportation, and other stabilization needs. This role will assist with reentry-focused programming and groups, depending on facility needs and program capacity.

Requirements

  • Two to five years of case management, social services, correctional, behavioral health, substance use, or reentry experience required.
  • Correctional, detention, jail, or community reentry experience required.
  • Experience working with justice-involved individuals, high-needs populations, behavioral health, substance use, or community-based service coordination experience required.
  • Clear and effective verbal and written communication with all coworkers, supervisors, jail administration, and patients
  • Excellent verbal and written comprehension
  • Excellent deductive reasoning and problem-solving abilities
  • Excellent organizational skills
  • Ability to use a computer and use/learn a variety of software, including site-specific computer programs
  • Ability to respect the dignity and confidentiality of inmates
  • Must maintain all certifications, educational requirements, licensing, etc. for this position
  • Must have current CPR/BLS certification
  • Must have current TB test (taken within one year)
  • Must adhere to all facility policies and procedures as well as the policies and procedures listed in the Employee Handbook
  • Willing to assist coworkers in the job duties and work overtime if required; may act as a resource to other coworkers
  • Maintains confidentiality, dignity, and security of health records and protected health information in compliance with HIPAA requirements
  • Must undergo security training and orientation on facility safety policies and procedures
  • Must be able to pass a background check and pre-employment drug test (as applicable).

Nice To Haves

  • Associate's degree in a health-related, human services, criminal justice, social work, psychology, counseling, or related field preferred.
  • Bachelor's or master's degree in social work, counseling, psychology, criminal justice, human services, or related field preferred.
  • A combination of education and experience will be reviewed on a case-by-case basis.
  • Certified Alcohol and Drug Counselor certification preferred.
  • Certified Peer Support Worker certification preferred.
  • Additional training in motivational interviewing, trauma-informed care, evidence-based reentry practices, MAT/MOUD, CBT-based programming, or case management preferred.

Responsibilities

  • Interviews inmates to provide preliminary discharge plans
  • Identifies who needs outside resources upon release
  • Electronically documents all interventions and patient response in patient's medical record
  • Assists in matching referrals to needed services
  • Assists in filling out required/relevant paperwork
  • Provides patients with information needed for aftercare services
  • Serves as liaison between County Alcohol, Drug, and Mental Health Office, County Public Defender's Office, and the Jail Mental and Medical units
  • Participates in case management meetings with on-site mental health providers
  • Participates in Quality Management meetings, Quality Management Peer Review activities, and staff training
  • Responsible for discharge planning for MAT patients in accordance with the policy and procedures
  • Plans, develops, and recommends priorities, goals, and long-range objectives pertaining to the management and direction of the detention facility
  • Collects and analyzes program and community data to assess community strengths and needs
  • Identifies participants and identifies community partners to meet the needs of patients and maximize the use of available community resources
  • Responsible for providing individual and/or group treatment for identified MAT patients who are scheduled to be released into the community
  • Assist with individual or group reentry/reintegration programming for identified individuals, within scope of role and under appropriate clinical oversight when applicable.
  • Assists with developing program brochures and other related program marketing items
  • Performs other duties as assigned
