About The Position
We’re on the hunt for a Hospice Liaison who knows how to build genuine relationships with referral sources, communicate compassionately with families, and never backs down from a good parking lot conversation at an assisted living facility.
Requirements
- Ability to build genuine relationships with referral sources.
- Ability to communicate compassionately with families.
- Ability to engage in conversations at assisted living facilities.
Nice To Haves
- Familiarity with SNFs (Skilled Nursing Facilities).
- Understanding that hospice is about showing up differently, not giving up.
- Having referral partners who communicate informally (e.g., texting memes).
Responsibilities
- Explaining hospice with empathy, warmth, and not sounding like a textbook.
- Juggling facility visits, calls, family meetings, and coffee runs like a pro.
- Being a familiar, friendly face to our referral partners.
