Requirements

  • High school diploma or equivalent required; Associate degree in a human service-related field preferred.
  • Minimum of one year of experience working with older adults and adults with disabilities preferred.
  • Strong public speaking and group facilitation skills.
  • Ability to work independently and manage multiple tasks.
  • Excellent organizational skills and ability to multi-task.
  • Strong verbal and written communication skills.
  • Strong interpersonal skills with older adults, adults with disabilities, and community members.
  • Knowledge of local services, agency services, and referral procedures.
  • Proficiency in computer and internet-based programs, including Microsoft Office applications.
  • Passage of complete background clearance and any other required testing.
  • Support for the Codes of Ethics established by Catholic Charities USA, NASW, and APA.

Responsibilities

  • Facilitate ACL-approved evidence-based health promotion programs (e.g., A Matter of Balance, Bingocize, Tai Ji Quan).
  • Organize and facilitate educational activities on topics such as nutrition, health, mental health, and personal care.
  • Organize and facilitate recreation activities to promote health and social well-being.
  • Complete health screening and evaluation services, including organizing health care activity events.
  • Conduct outreach to high-risk or vulnerable populations.
  • Participate in community outreach and participant recruitment activities.
  • Maintain distribution lists for community sites to promote programming.
  • Ensure timely and accurate completion of required documentation in internal and partner systems and databases.
  • Collects surveys and outcome information from participants in Community Based Programming.
  • Maintain cooperative relationships with service providers and community organizations.
  • Act as an agency ambassador and attend community groups, meetings, and events as assigned.
  • May be required to offer health screenings or demonstrations of programs at these events.
  • Develops and plans educational and recreation programming for counties served including the purchase of materials as approved by Program Manager.
  • Timely and accurate submission of all programmatic expenses and documentation, including receipts and mileage logs for Agency Vehicle.
  • Assists with vehicle maintenance routines.
  • Other duties as assigned within the guidelines of this position.
