About The Position

OLV Human Services is looking for a detail-oriented and motivated part time Community Provider to join our dynamic team! The Children’s Community Provider provides direct services to children and families in their community, including the CFTSS and HCBS programs. They will foster the highest possible quality of life and personal achievement in accordance with the individual’s person-centered plan.

Requirements

Ability to work as part of a team, as well as independently.

Ability to navigate the internet and basic computer operations to include use of EHR and video conferencing technology.
Organizational and time management skills.
Competency/ Knowledge Areas: Youth ages 0-21-mental, developmental, behavioral.
Training/ teaching skills; Community resources.
New hire orientation (including Infection prevention, HIPPA compliance, Corporate Compliance, fire safety).
Risking Connections Trauma Training.
Diversity Training.
Autism awareness.
Safety in the field.
Suicide prevention.
Strength Based approaches (including Person Center Care).
Domestic Violence.
Stages of development in children.
Valid NYS Driver’s License.
High School education with 2 years’ experience in Mental Health, Substance Abuse Disorder, or Foster Care OR Bachelor's Degree in Human Services.

Responsibilities

Providing support services to youth with a range of emotional, behavioral and social difficulties.
Encouraging growth and recognizing the autonomy of each individual.
Attending paid new worker training and ensuring regulatory training standards are met.
Providing required face-to-face services in the community or home, requires use of personal vehicle.
Attending team meetings, supervision sessions, staff meetings as scheduled.
Forming and maintaining relationships with youth and caregivers.
Working with youth to achieve goals consistent with their service plans.
Written communication skills for documentation, including progress notes, correspondence notes, service plans, and safety plans which show progress toward individual goals, observations and interactions.
Demonstrating competency with navigating electronic health record (EHR) for documentation of services rendered.
Reviewing utilization and service provision with supervisor.
Coordinating and collaborating with Care Managers to identify service needs and develop service plans.
Ensuring appropriate chain of command is utilized.
Complying with all federal and state regulations and agency policies pertaining to HIPAA, false claims, whistleblower policies and all others.
Performing any other duty as assigned by your supervisor.
