About The Position

AS PART OF OUR MOBILE INTEGRATED HEALTHCARE TEAM, THE COMMUNITY PARAMEDIC PROVIDES HIGH-QUALITY, COMPASSIONATE IN-HOME AND COMMUNITY-BASED CARE TO PATIENTS WHO MAY NOT REQUIRE EMERGENCY TRANSPORT BUT NEED ACUTE OR NON-ACUTE TREATMENT IN PLACE, ONGOING CARE, OR FOLLOW-UP SUPPORT. WORKING IN COLLABORATION WITH PRIMARY CARE PROVIDERS, EMERGENCY DEPARTMENTS, PHYSICIAN PRACTICES, HOSPITAL SYSTEMS, SOCIAL SERVICES, AND BROADER HEALTHCARE TEAMS, THE COMMUNITY PARAMEDIC HELPS REDUCE UNNECESSARY EMERGENCY ROOM VISITS, IMPROVE PATIENT OUTCOMES, AND SUPPORT CHRONIC DISEASE MANAGEMENT, PREVENTIVE CARE, AND POST-HOSPITALIZATION FOLLOW-UP. THIS ROLE PERFORMS ALL FUNCTIONS OF AN EMPRESS EMS NYS EMT-PARAMEDIC.

Requirements

  • Certified Paramedic (NREMT-P or NYS EMT-P)
  • BLS, ACLS, and PALS certification required
  • High school diploma or GED required
  • Minimum of 2 years of experience as a field paramedic
  • Strong interpersonal and communication skills
  • Ability to work independently and collaboratively across diverse environments
  • Knowledge of chronic disease management and public health principles
  • Comfort with Electronic Medical Records (EMRs) and mobile health technology
  • Strong problem-solving and clinical decision-making skills in non-traditional settings
  • Cultural sensitivity and the ability to work with diverse populations

Nice To Haves

  • Completion of a Community Paramedicine Program (training will be provided for qualified applicants)
  • Experience in community health, primary care, or public health is an asset

Responsibilities

  • Conduct advanced in-home patient assessments, including physical exams, medication review, safety evaluations, and health education
  • Provide urgent and unscheduled patient medical responses
  • Deliver treatment in place through established protocols and physician collaboration
  • Support chronic disease management (e.g., diabetes, heart failure, COPD)
  • Follow up with recently discharged patients to ensure recovery and adherence to discharge plans
  • Provide ongoing clinical monitoring and interventions, escalating care when needed
  • Assist patients with access to social and community services
  • Collaborate with healthcare providers to implement and support patient care plans
  • Maintain accurate, timely documentation of all assessments, interventions, and referrals
  • Participate in public health initiatives, health screenings, and community outreach programs
  • Educate and coach patients and families on health maintenance, disease prevention, and self-care
  • Respond to 911 emergency calls when appropriate to support the EMS system
