Acadian Health is seeking an experienced Community Paramedic to provide hospital-level care in patients' homes as part of our Hospital-at-Home program. This role combines advanced clinical skills, care coordination, patient education, and technology to support patients recovering from acute illnesses and medical conditions outside of a traditional hospital setting. The Community Paramedic serves as the in-home clinical partner to a virtual care team, helping ensure safe, effective, and high-quality patient care. The ideal candidate is an experienced paramedic who is comfortable working independently in patients' homes, collaborating virtually with healthcare teams, and providing advanced patient care outside of a traditional hospital setting. Community Paramedicine, Critical Care, or Mobile Integrated Healthcare experience is highly valued. This position requires current Paramedic licensure and experience providing direct patient care in emergency, acute care, or pre-hospital settings. Candidates who are not currently licensed as a Paramedic or who lack field-based patient care experience may not be considered.
