About The Position

This part-time benefitted position performs all duties as described under the position description Paramedic. The Community Paramedic conducts follow-ups with referrals from internal and external stakeholders to reduce 911 system utilization and recurrent emergency department visits. This role involves educating patients on medicine and disease processes, completing various assessments, documenting findings on appropriate software, and communicating relevant findings to providers. The Community Paramedic will coordinate and refer patients to various health care and social services, participate in community outreach events, and respond to 911 calls for service as dispatched. Work is performed under the limited supervision of the Community Paramedic Program Manager, with a specific focus on Opioid/Substance Use Disorder. This role provides proactive care to improve the overall health and wellness of program participants, expanding access to non-urgent healthcare through in-home patient visits and community-based services. Services include assessment, treatment, prevention, and referral, aiming to produce better health outcomes and reduce ambulance transports, emergency department visits, and hospital readmissions. The Community Paramedic acts as a "navigator" of community resources, connecting patients to services like Substance Use Counseling, Sober Living Facilities, and Medication Assisted Treatment Centers, thereby reducing barriers to care. The role also involves outreach education to the public, documenting visits in an electronic medical record, and entering required information into a client database for statistical analysis. The Community Paramedic will also act as a 911 Field Staff Paramedic as needed, maintaining all necessary knowledge, skills, and competencies. This includes attending conferences, training classes, and meetings to stay current with laws, rules, and regulations, providing stand-by coverage at special events, and participating in informal and in-service training to maintain certifications. The role also involves participating in community outreach programs, conducting First Aid and CPR classes, attending community events to promote safety and prevention, and attending staff and committee meetings as required.

Requirements

Possession of an associate's degree in emergency medical services or similar field, including completion of an accredited EMS program; or an equivalent combination of education and experience.
Current certification through N.C. Office of Emergency Medical Services as a Paramedic or ability for reciprocity.
Must possess and maintain valid N.C. Driver’s License without restriction, with the exception of corrective lenses.
Must possess and maintain a good driving record.
Must be physically able to lift approximately 250 lbs. of share weight.
Ability to operate loaded ambulance stretcher.
2 years of experience as a functioning Paramedic.
4 years of EMS experience.
Ability to function independently with limited oversight from superiors.
Requires excellent rapport, understanding, and communication with the patients and their family members.

Functions as physician and hospital extenders by utilizing advanced protocols/procedures approved by the Medical Director.
Provides outreach education to the public (i.e. Hands-only CPR, School career days, etc.).
Documents visit in electronic medical record.
Enters required information into client database in order to collect and analyze statistics for evaluation plan.
Acts in the capacity of a 911 Field Staff Paramedic as needed.
Maintains all knowledge, skills, and competencies to perform this role.
Attends conferences, training classes, meetings to remain current in laws, rules and regulations.
Provides stand-by coverage at special events.
Participates in informal training activities and informal in-service training classes to maintain required certifications; attends other training opportunities.
Participates in community outreach programs; conducts training in First Aid and CPR classes; attends programs such as community events to promote safety and prevention awareness and to provide information on services.
Attends in-service classes to maintain current knowledge of emergency medical technology and procedures.
Attends staff meeting and committee meetings as required.
Performs other related duties as require
