About The Position

Orange County Healthy Families is a home visiting program designed to support new and expectant parent and caregivers as a means to reduce stress and bond with their child. The Outreach Specialist is responsible for generating referrals for the Orange County Healthy Families program. The Outreach Specialists goal is to conduct outreach to community providers and potential program participants to increase program capacity.

Requirements

  • Excellent computer skills and a comprehensive knowledge of Outlook, Microsoft and data entry.
  • Flexible schedule including working evenings and possible weekend events.
  • Effective written and communication skills.
  • Valid unrestricted driver's license required.
  • Must have reliable transportation.
  • High School Diploma or Equivalent.
  • Experience with families and children.

Nice To Haves

  • Bi-lingual English/Spanish speaking preferred.

Responsibilities

  • Builds positive working relationships with community service providers.
  • Represents the program on community panels and conducts regular community outreach.
  • Maintains existing relationships and develops new relationships with referral sources such as OBGYN, WIC, health clinics, hospitals and community based organizations.
  • Becomes knowledgeable in Healthy Families program and becomes efficient in communicating the program and its benefits to community members, professionals and referred families.
  • Works with a small caseload of families and conducting home visits.
  • Engages families and develops relationships based on a strength-based model.
  • Introduces and leads parent-child activities that support healthy attachment and foster optimal child development.
  • Provides daily support and crisis management skills to families.
  • Partners with families to create a plan with attainable goals to enhance parent and child well-being.
  • Maintains thorough written records of home visits, family goals, and all required documentation.
  • Attends mandatory Healthy Families trainings and completes within the required time frame.
  • Completes ongoing trainings as identified by Supervisor.
  • Participates in weekly supervision and regular team meetings.
