About The Position

The Community Outreach Specialist works in collaboration with staff, volunteers, and interns in the execution of assigned tasks including but not exclusive of service coordination, comprised of intake screening, needs assessment, information and referral, community linkage, and outcomes follow up. This work is conducted in a variety of settings: satellite locations and community events throughout Brooklyn & Queens. Responsibilities include but are not limited to: adherence to all administrative and data collection policies, collaboration with internal and external networks, and engagement in community building activities in support of the agency’s mission. This work will be completed within the model of care which is holistic, family centered, strengths-based, and serves to empower clients and enrich the community. The agency has embraced the concept of a fully integrated system of care for all consumers of service. This requires that the staff assess the client's needs holistically and see it as their responsibility to connect the client with services needed--internal or external to the agency, regardless of the door to which the consumer enters the system.

Requirements

  • Bachelor’s degree in Social Work, Human Services, or related field.
  • Proficient in professional social work generalist practice skills.
  • Excellent communication (written & oral), interpersonal, and transactional skills.
  • Frequent sitting, standing, walking, & climbing stairs.
  • Ability to lift 30 lbs.
  • Ability to work flexible hours (evenings/weekends) as assigned.
  • Able to travel throughout the five boroughs as needed.

Nice To Haves

  • 2 years of experience working with low income families is preferred.
  • Bilingual English/Spanish, English/Arabic, English/Cantonese/Mandarin, and/or English/ Russian preferred.

Responsibilities

  • Travel to various sites throughout Brooklyn and Queens to provide service coordination as assigned.
  • Provide direct service coordination to clients, including intake documentation, needs assessment, service/referral planning and linkage, and follow-up on referral outcomes.
  • Works with the Office of Mission to respond to emergency community needs.
  • Assess eligibility for and facilitate the distribution of available funds from Emergency Food and Shelter Program (EFSP), and other grants/funding sources, to financially assist clients.
  • Develop internal and external linkages while engaging in community building activities and promoting program services through outreach initiatives.
  • Ensure timely, accurate and thorough documentation of services and referrals in agency database
  • Track emergency funds assistance and utilization in agency database.
  • Work in partnership with Call Center staff to provide high quality and timely access to services to all clients entering the agency.
  • Participates in program planning, staff meetings, case reviews, and group services.
  • Assists in development of on and off-site events affiliated with food pantries, parishes and other community event sites, and works in collaboration with Parish and Community Relations Directors on such, as needed.
