About The Position
Executive Home Care of Omaha is seeking a motivated, relationship-focused professional to build referral partnerships with skilled nursing facilities, rehabilitation centers, assisted living communities, and memory care facilities throughout Omaha. This role involves helping seniors and families access quality in-home care while building valuable partnerships in the healthcare community.
Requirements
- 5+ years of experience in social work, discharge planning, or a related field.
- Knowledge of the Omaha healthcare and senior care community preferred.
- Strong communication and relationship-building skills.
- Self-motivated with reliable transportation.
Responsibilities
- Build relationships with social workers, discharge planners, and case managers.
- Visit healthcare facilities and introduce our in-home care services.
- Develop referral partnerships and follow up with prospective partners.
- Track outreach activities and referral opportunities.
