On our team, you will provide stellar care at one of the top healthcare organizations in the nation. As a Community Outreach Patient Navigator supporting the Trauma department, you will serve the community by reducing health disparities through patient navigation, education, and coordinated support across the continuum of care. In this role, you will screen all new trauma admissions, perform mental and social health assessments, educate patients and families on injuries, treatment, recovery, prevention, and available services, and coordinate appointments and care needs. You will remove barriers to care such as insurance, financial, transportation, knowledge, and psychosocial challenges while connecting patients and families to resources within and outside of the Cleveland Clinic. A caregiver in this role works days from 8:00 a.m. to 4:30 a.m. This position is based at Fairview Hospital, with additional locations as needed, and hours may vary.

Requirements

  • High School Diploma or GED
  • Three years of related experience in customer relations with diverse patient/culture populations, exhibiting problem solving skills
  • Excellent patient service orientation skills
  • Interpersonal skills with special emphasis on oral and written communication, process improvement and problem resolution
  • May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating.
  • May need to remain still for extended periods, including sitting and standing.
  • May need to lift boxes or carry objects
  • Working evenings and weekends is required
  • All offers of employment are followed by testing for controlled substances.
  • You will be required to comply with this program, which will include obtaining an influenza vaccination on an annual basis or obtaining an approved exemption.

Nice To Haves

  • Knowledge of Electronic Medical Record (EMR)
  • Proficient in spoken and written Spanish
  • Prior experience in healthcare
  • Skills related to social worker or care management
  • Counseling experience

Responsibilities

  • Coordinate and educate patients within the community on disease prevention and screening.
  • Identify and address barriers to care, including fear, trust, financial, insurance, transportation and psychosocial concerns, to ensure appropriate clinical care is coordinated and provided in a timely manner.
  • Help patients navigate our complex health care systems.
  • Coordinate appointments related to health screenings, such as mammogram results, colonoscopy results and pathology reports.
  • Navigate patients to appropriate health care providers within Cleveland area facilities.
  • Identify barriers to care and coordinate resolution through social work support, Medicaid, Medicare, prescription assistance programs, community resources, primary care services and medical homes.
  • Support the coordination and execution of community events.
