Job Preview at a Glance: Job is a specialty position working with vulnerable individuals and families of all ages suffering from Opioid Use Disorder (OUD) and its effects on those who are uninsured or under insured. Works with the multiple disciplines and persons that make up the treatment team. Provides all direct care services associated and is fluent in processes and available resources. Position works independently, carries a caseload, and provides an appropriate level of service to the community and individuals in planning treatment-based alternatives to continued use. Requires interface with the persons served, community partners, community resources, families, integrated service providers, SB&H staff, programs, and supervisors on a regular and daily basis. Provides individual client contact and service provision, assessment, service planning, case management, crisis and behavioral health interventions/services, referral, and follow up. Position works as a partner to the system and partner to persons served to ensure success. Locations and Schedule: This position reports to our Flagstaff team!! Must have flexible availability to attend Community Events. Pay: $19-$20/hr. This is a grant funded position.
