About The Position
Essen Health Care is the largest privately held, multispecialty medical group in New York, providing high-quality, compassionate care to some of the state’s most vulnerable and underserved residents. Founded in 1999, we’ve grown from a single primary care office into a network of 50+ locations offering urgent care, primary care and specialty services, from women’s health to endocrinology and psychiatry. We also provide nursing home support, care management, and in-home care through our Essen House Calls program. Requirements
- Minimum High School Diploma or GED required
- Previous experience in community outreach, customer service, healthcare, social services, or patient engagement preferred
- Strong interpersonal and communication skills
- Ability to work independently and in a team environment
- Bilingual skills strongly preferred (Spanish, Russian, Chinese, or other languages a plus)
- Comfortable working in the field and traveling locally throughout NYC boroughs
- Basic computer skills including Microsoft Office and email communication

Responsibilities
- Conduct community outreach activities to promote Essen Health Care services and programs
- Build and maintain relationships with community organizations, local businesses, schools, senior centers, shelters, and other partners
- Attend health fairs, community events, and outreach initiatives on behalf of Essen Health Care
- Educate community members on available healthcare services, insurance resources, and wellness programs
- Assist patients with scheduling appointments and connecting to appropriate departments
- Support membership growth and patient retention initiatives
- Maintain outreach logs, event tracking, and community engagement reports
- Distribute marketing and educational materials within the community
- Collaborate closely with operations, marketing, care management, and clinical teams
- Represent Essen Health Care professionally and positively in all community interactions
