About The Position

Join Via Care as a community Outreach and Enrollment Specialist and help make healthcare more accessible to the communities we serve. In this community-facing role, you will foster community relationships, attend outreach events, educate residents about available healthcare services, and support patients through the enrollment and onboarding process. The Outreach and Enrollment Specialist plays a key role in expanding access to care throughout Via Care's service areas by promoting services, strengthening community partnerships, and connecting individuals and families to primary care, dental, behavioral health, women's health, senior services, and urgent care. Working closely with internal teams and community partners, this position helps remove barriers to care, increase patient engagement, and support healthier communities.

Requirements

  • Bilingual in English and Spanish.
  • Experience working with culturally diverse, underserved, and low-income communities, including predominantly Latino populations.
  • Ability to interact effectively with individuals from diverse backgrounds while demonstrating professionalism, respect, and cultural sensitivity.
  • Ability to work independently, manage multiple priorities, and thrive in a fast-paced environment.
  • Must have a reliable automobile for use on the job (mileage to be reimbursed).
  • Valid California Driver License and automobile insurance coverage.
  • Subject to a criminal background check prior to employment.
  • TB clearance, to be renewed every year.
  • Vaccines and boosters as needed.
  • Compliance with all mandated vaccinations and all boosters is a term and condition of employment.

Nice To Haves

  • Two (2) years of experience in healthcare, community outreach, enrollment services, public health, or social services.
  • Covered California Enrollment Assister Certification strongly preferred.
  • Current HIV/STI Counseling Certification and training in family planning services.

Responsibilities

  • Attend, organize, and represent Via Care at community events, health fairs, and school or senior center functions.
  • Present clinic services to a wide range of stakeholders, including schools, local businesses, nonprofits, faith-based groups, and housing authorities.
  • Distribute educational materials and communicate accurate information regarding Via Care services and programs.
  • Build and maintain strong partnerships with community organizations, agencies, advocacy groups, and referral sources.
  • Identify and develop new outreach opportunities to increase awareness of and access to healthcare services.
  • Educate community members about available healthcare services and assist individuals in establishing care with Via Care.
  • Support patient enrollment, registration, appointment scheduling, and onboarding activities.
  • Coordinate closely with enrollment staff, mobile units, and clinic operations teams to promote seamless onboarding and continuity of services.
  • Conduct outreach and follow-up efforts to increase appointment completion and patient engagement.
  • Track and report outreach activities, community partnerships, patient engagement rates, enrollment appointments, and referral outcomes.
  • Maintain accurate records of outreach contacts, events, and community engagement activities.
  • Travel throughout Via Care's service areas, including East Los Angeles and surrounding communities, to support outreach and enrollment efforts.
  • Attend occasional evening or weekend community events as needed.
