About The Position

Under the direct supervision of the Mobile Health Services Program Supervisor, the Community Outreach Educator will assist in three main activities: (i) provide vaccine education and community engagement, focusing on hard-to-reach communities and populations; (ii) recruitment of new program participants and partners; and (iii) conduct culturally appropriate outreach to promote department services among Hispanic, Black/African American, LGBTQ+, rural, and other vulnerable populations, as well as outreach for the Mobile Vaccination Services (MVS) Program under the Mobile Health Services Department (MHS). The Community Outreach Educator will support the MVS program in educating SYHealth patients and community members on the importance of the MVS Program, merging SYHealth’s immunization services under the Mobile Health Services Department. The responsibilities of the Community Outreach Educator will include providing education and outreach related to the MVS program; in-person outreach (door-to-door, canvasing, tabling) focusing on the hard-to-reach community and at SYHealth mobile clinics locations. Furthermore, the Community Outreach Educator will be responsible for the registration and referral of potential participants by creation of appointments and referrals; providing general program information; tracking outreach/engagement activities for reporting purposes; and supporting data collection processes. The Community Outreach Educator will also be responsible for other duties necessary for the successful implementation and completion of the Mobile Vaccination Services Program as assigned by the Mobile Health Services Program Supervisor and Mobile Health Services Manager. The Community Outreach Educator key responsibilities will include finding new community partners, coordinating outreach efforts before and during Mobile Health Service events, implementing outreach strategy for events in alignment with best practices; attending outreach events with proper equipment, collaborating with external affairs team to promote Mobile Health Services via San Ysidro Health social media sites, taking pictures of Mobile Unit related events to promote Mobile Health services, collaborating with other team members to ensure the effective functioning of department activities and efficient use of program resources; supporting monitoring and tracking of outreach impact in relation to services provided including documentation and service tracking; working with the Program Supervisor to assure quality improvement goals/objectives and policies/procedures are enforced, and other duties as assigned.

Requirements

  • 2-years’ demonstrate experience working with community-based organizations in community outreach and engagement, health promotion, customer services with especial emphasis in Hispanic and Black/African American communities
  • High School Diploma or GED Equivalent
  • Excellent oral and written communication skills
  • Is an active listener and provides support and guidance to others
  • Superb organizational ability and outstanding social engagement and customer service skills
  • Familiarity with excel and database management software
  • Prolonged, extensive, and considerable standing/sitting/walking/lifting
  • Manual dexterity and mobility
  • Always reaching, stooping bending, kneeling, crouching
  • Good organizational skills and ability to remain focused and concentrate with noise around
  • Ability to handle multiple job task functions simultaneously

  • Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.

Responsibilities

  • Conducts culturally appropriate outreach to promote department among Hispanic, Black/African American, LGBTQ+, rural and other vulnerable populations
  • Responsible for development of materials and tools necessary to appropriately market, promote and track all department events, services and outreach involved with program goals
  • Attends various outreach and engagement events in both community and clinical settings as assigned by supervisor
  • Assist in the setup, operation, and maintenance of research engagement/outreach equipment as specifically instructed
  • Recruits and refers to potential participants/ partners to department services
  • Conducts follow-up calls/ emails to referred/interested potential participants or partners
  • Acts as liaison with community partners and collaborators across of San Diego County to identify event locations and coordinate and implement vaccination community events
  • Obtains verbal and written consent to collect demographic variables/ feedback forms from participants
  • Performs data entry using the study data management system (i.e.; Excel, EPIC)
  • Provide reports and data summaries relevant to outreach activities as requested by the Program Supervisor or Manager
  • Assists with routine data verification and quality controls to ensure data integrity and consistency
  • Aid in a variety of tasks in support of established recruitment and retention goals.
  • Provide assistance to the Program Manager, and Program Supervisor as needed
  • Enhances professional growth and development through participation in educational programs
  • Attends meetings as required and participates in committees as directed
  • Performs other duties assigned
