HealthRIGHT 360’s Mobile and Street Medicine program delivers comprehensive, high-quality medical care and social services to individuals experiencing homelessness in San Francisco. Rooted in the mission to "go to the people", the program offers services directly to patients—on the street, in encampments, or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC), expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier, trauma-informed, and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust, continuity, and access while delivering compassionate, dignified, and high-quality services to some of the city’s most underserved populations. The Community Outreach Driver is a key member of the mobile medicine team, responsible for supporting community outreach, harm reduction, and transportation services. This role involves working as part of a multidisciplinary team to engage individuals experiencing homelessness and connect them to essential medical and social services aimed at improving health and quality of life. The Community Outreach Driver will operate the mobile medical van, provide safe transportation for the outreach team, and support team safety in the field. Additionally, this role includes direct engagement with clients, offering harm reduction education and supplies, and assisting with basic case management tasks in collaboration with the case managers. As a trusted presence in the community, the Outreach Driver also serves as a "street guide," helping the team navigate encampments and other field settings effectively and respectfully.
