A few details about the role: The Outreach Coordinator is responsible for developing and maintaining relationships with healthcare providers, community organizations, and families to promote the facility's services for Health Center and Assisted Living. This role plays a key part in increasing census through strategic outreach, education, and referral development. Looking for a team-oriented Sales Professional that has experience with collaborating with operations and nursing to grow occupancy. Perform public relations, marketing and development for the community as needed. Responsible for maintaining census at budgeted level. Serves as public relations agent on behalf of the community with referring communities, soliciting feedback, and sharing information with supervisor. Make presentations about the community at scheduled physician networking luncheons. Work closely with health center and social service departments to facilitate efficient admission/transfer to HC from referring community to appropriate level of care.

Requirements

  • Associates or bachelor's degree required.
  • Two-three years applicable experience.

Responsibilities

  • Develop and execute outreach strategies to increase awareness of the community's services.
  • Build and maintain relationships with hospitals, physicians, discharge planners, case managers, and other referral sources.
  • Represent the community as a whole at community events, health fairs, senior expos, and networking meetings.
  • Conduct tours of the healthcare center and assisted living for prospective residents, families, and referral partners.
  • Collaborate with the admissions and nursing teams to ensure smooth transitions for new residents.
  • Track outreach activities, referral sources, and conversion rates using CRM tools and use CRM to review leads for Assisted Living and documentation for both Health Center and Assisted Living.
  • Provide education about assisted living and skilled nursing services to community partners.
