About The Position

The Community Outreach Coordinator plays a key role in supporting Food Bank of Central New York’s Health-Related Social Needs (HRSN) initiatives. This position conducts screening and navigation services with individuals as part of the Food Bank’s SCN contracts, provides administrative and logistical support for HRSN referrals, maintains effective internal communication, and coordinates food box distributions for program members. The role ensures timely and accurate referral processing in nine of the Food Bank’s eleven county service area. This is a grant funded position.

Requirements

  • Associate’s or Bachelor’s degree in public health, social work, community nutrition, or a related field preferred.
  • Valid driver’s license and ability to travel within the nine-county service area as needed.
  • This position will receive a pre-employment background check.
  • Food Bank of Central New York complies with all applicable state and federal laws pertaining to drug testing and background checks, including Article 23-A.
  • 1–2 years in a community outreach, case coordination, or program support role.
  • Comfortable using data systems, spreadsheets, and digital communication platforms.
  • Strong organizational and time-management skills with keen attention to detail.
  • Excellent verbal and written communication skills.
  • Ability to manage multiple priorities in a fast-paced environment.

Nice To Haves

  • Familiarity with food insecurity and social determinants of health is a plus.
  • Bi-or multi-lingual preferred.

Responsibilities

  • Complete screenings using the required tool with individuals receiving Medicaid.
  • Refer interested individuals to services within and outside of the Social Care Network.
  • Stay current on required trainings and compliance expectations outlined by SCN leads.
  • Maintain accurate records and documentation for each individual served, including interaction notes.
  • Assist with referrals for assigned services, including tracking, organizing, and responding in a timely manner.
  • Coordinate internally and externally to ensure referral accuracy and appropriate follow-up.
  • Provide administrative support for HRSN services, including maintaining internal communication, collecting and organizing data for reporting and evaluation, supporting scheduling and communication for programming and outreach, and other related tasks.
  • Assist with the preparation and coordination of food box distributions for members.
  • Collaborate with operations and distribution teams to ensure timely deliveries across counties.
  • Monitor inventory, distribution metrics, and other relevant or assigned KPIs.
  • Represent the Food Bank at meetings and outreach events as needed.
  • Support education and awareness efforts related to available HRSN services.
