PanCare of Florida is seeking a skilled and compassionate Community Outreach Coordinator to join our team, supporting our Ryan White program. Requirements
- 2+ years' experience in community outreach, social services, public health, or a related field required.
- Experience working with underserved populations or individuals affected by HIV
- Familiarity with the Ryan White Program, HIV care navigation, or case management (preferred but not required)
Responsibilities
- Conduct outreach and education for individuals living with or at risk for HIV
- Provide Rapid HIV testing and ensure follow-up care for individuals testing positive
- Support individuals with counseling, linkage to care, and retention services
- Re-engage individuals who have missed appointments or fallen out of care
- Distribute health education materials and prevention resources
- Document testing, outreach, and program services accurately
- Build relationships with community organizations and healthcare partners
- Assist case managers with referrals, intake support, and client documentation
- Attend community events, health fairs, and outreach opportunities (some evenings/weekends may be required)
