About The Position

Nurses working for CarePartners in the community experience flexible scheduling and work-life balance. Working within a supportive and collaborative environment focused on quality patient care, community nurses can exercise their full scope of practice, pursue specialization, and champion innovative health care programs. This is a Part-Time position in Chatham-Kent County, with hours of work being OVERNIGHTS within a 2-week rotation, inclusive of weekends and weekdays. We are looking for RNs and RPNs who will be working with Pediatrics and other patients.

Requirements

Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
Current College of Nurses of Ontario (CNO) registration in good standing
A strong desire and commitment to making a difference in the lives of our patients
Exceptional interpersonal skills
A valid driver’s license and vehicle as regular travel is required
Excellent English verbal and written communication skills
CPR Certification
Current immunization record including 2 step TB test
A current vulnerable sector check and a clear background check

Responsibilities

Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
Working with patients (both in-person and virtually) to provide exceptional nursing services.
Providing services in a variety of settings which may include in-home, clinic, retirement home or school.
Communicating and liaising with patients, family members and others within the interdisciplinary care team.
Performing other clinical duties as required.
