We are looking for RNs and RPNs to work as community nurses servicing patients both in home as well as in the clinic. We have full time and part time positions available. Hours of work are inclusive of Weekdays/ Weekends & on-call rotation. CarePartners Participates in the CCPN Grant - Ontario Health- HFO, some nurses may qualify, speak to your recruiter about eligibility of the $25,000 HFO Grant.

Requirements

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • A valid driver’s license and vehicle as regular travel is required
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check

Responsibilities

  • Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Working with patients (both in-person and virtually) to provide exceptional nursing services
  • Providing services in a variety of settings which may include in-home, clinic, retirement home or school
  • Communicating and liaising with patients, family members and others within the interdisciplinary care team
  • Performing other clinical duties as required
