This specific Part-Time position is located in the Woodstock Transition Care Unit and involves supporting patients who require additional care while awaiting a Long-Term Care plan or rehabilitation to home. The role is open to Registered Nurses (RNs) and Registered Practical Nurses (RPNs) seeking flexible schedules.

Requirements

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • Current vulnerable sector check and a clear background check

Responsibilities

  • Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Working with patients to provide exceptional nursing services
  • Providing services in a variety of settings which may include in-home, clinic, retirement home or school
  • Communicating and liaising with patients, family members and others within the interdisciplinary care team
  • Performing other clinical duties as required
