About The Position

We have a rewarding opportunity for a Community Navigator to join the team within our Cambridgeshire Navigators Service. Our Community Navigator Team offer person centred support to clients to help them live well and achieve their individual outcomes. The team use their local knowledge and expertise to link people in with services and provide advice, information, guidance and signposting. As the Community Navigator, your role will aim to address the rising needs of adults in Cambridgeshire by connecting them with local services, community groups, and emotional support to reduce isolation, enhance mental health, and support independent living. The role will aim to address the needs of adults in East Cambs & Fenland by offering information and guidance so clients can explore their options. You will provide one to one guidance and information for individuals needs using a range of methods including face to face appointments, telephone support, virtual appointments and via email/text. Requirements

  • NVQ level 2 (or equivalent) in Health and Social Care or willingness to work towards this.
  • Previous experience of working with individuals to assess their needs and produce plans to meet outcomes is essential for this role.
  • Candidates personal values align with Making Space values.

Responsibilities

  • Address the rising needs of adults in Cambridgeshire by connecting them with local services, community groups, and emotional support to reduce isolation, enhance mental health, and support independent living.
  • Address the needs of adults in East Cambs & Fenland by offering information and guidance so clients can explore their options.
  • Provide one to one guidance and information for individuals needs using a range of methods including face to face appointments, telephone support, virtual appointments and via email/text.
