About The Position

The Community MAT Coordinator is responsible for building and maintaining strong referral networks, promoting Medication-Assisted Treatment (MAT) services, and supporting the admissions process for clients. This role works closely with hospitals, healthcare professionals, criminal justice partners, and community organizations to increase access to treatment and reduce barriers to care. The MAT Coordinator conducts outreach, delivers education and stigma-reduction initiatives, manages referral data, and ensures accurate documentation. Additional responsibilities include participating in quality improvement efforts, maintaining compliance with agency and regulatory standards, and engaging in ongoing professional development to stay current in addiction and mental health services. The position plays a key role in increasing client census, strengthening community partnerships, and supporting the overall mission of the organization.

Requirements

  • High School Diploma or GED
  • Certification in First Aid and CPR

  • Bachelor’s degree in psychology, social work, counseling or nursing from an accredited college or university.

Responsibilities

  • Build and maintain strong relationships with hospitals, physicians, social workers, case managers, and community organizations.
  • Develop formal partnership agreements with healthcare, criminal justice, and community agencies.
  • Deliver evidence-based education and training to improve referral practices and reduce treatment barriers.
  • Create and distribute stigma-reducing educational materials about MAT services.
  • Manage referral leads and coordinate the client admissions process.
  • Expand and strengthen referral networks within facilities and the community.
  • Collaborate with the clinical assessment team to ensure appropriate referrals for admission.
  • Educate patients, families, and community partners about available services and program offerings.
  • Travel within the community to conduct outreach, schedule meetings, and develop referral sources.
  • Document outreach efforts and referral contacts in the designated tracking system.
  • Assist with the development of events, trainings, and forums to promote programs and identify new referral partners.
  • Use analytical tools to monitor referral activity and outreach outcomes.
  • Represent the organization in professional forums and communicate program mission and goals.
  • Maintain consistent outreach communication through calls, emails, written materials, and networking activities.
  • Meet weekly productivity expectations and maintain an organized outreach schedule.
  • Provide admissions teams with required information to support efficient client intake.
  • Achieve performance benchmarks, including a 10% monthly increase in active client census.
  • Follow agency policies, procedures, and state/federal regulations.
  • Contribute to quality improvement activities and assist with data collection and analysis.
  • Maintain confidentiality, boundaries, and ethical standards with clients and colleagues.
  • Participate in professional development, continuing education, and required annual trainings.
  • Complete accurate and timely documentation and reports.
  • Perform additional duties as assigned.
