About The Position

The Community Living Manager (CLM) is responsible for resource development for individuals served, ensuring their life goals are implemented, personal outcomes are achieved, and the quality of care is high. The CLM assists in problem-solving and communicates regularly with program individuals, Direct Support Professionals (DSPs), and families across two locations. This role involves supervising DSPs, managing staffing and schedules, and providing coverage as a DSP when necessary. The CLM ensures compliance with all local, state, and federal requirements, as well as UCP's policies and procedures. Key duties include scheduling and facilitating meetings, overseeing compliance training, arranging activities, managing medication counts and MAR reviews, inspecting adaptive equipment, ensuring vehicle safety, maintaining home cleanliness and sanitation, managing grocery orders and menus, assisting with personal shopping, handling financial logs and reimbursements, ensuring accurate documentation, performing home inspections, coordinating maintenance, building relationships with stakeholders, ensuring completion of evacuation drills and safety preparedness, tracking behavior, managing medical appointments and transportation, and performing other assigned duties.

Requirements

  • Valid driver's license
  • CPR/FA certification
  • CPI certification
  • A High School Diploma
  • 5 years of experience as a DSP
  • At least 5 years of supervisory experience
  • Excellent verbal and written communication skills
  • Excellent interpersonal and customer service skills
  • Ability to lead and manage teams
  • Excellent organizational skills and attention to detail
  • Excellent time management skills with a proven ability to meet deadlines

Responsibilities

  • Lead, direct and manage a team of Direct Support Professionals.
  • Manage staffing and schedules, and find coverage when needed.
  • Schedule and facilitate both one-on-one and team meetings.
  • Provide coverage and function as a DSP in the absence of staff.
  • Partner with Human Resources on hiring and termination decisions; conduct reviews of applications.
  • Ensure that locations and staff operate within all local, state, and federal requirements.
  • Ensure staff and locations operate as outlined in UCP’s policies and procedures.
  • Ensure completion of compliance training and compliance activities in Relias, and follow up as needed.
  • Administer support or discipline for incomplete activities.
  • Schedule, arrange, and engage in activities.
  • Conduct weekly medication counts and MAR review.
  • Inspect and report on the status of adaptive equipment.
  • Ensure vehicles are operational and have required straps and tie downs for safe transport, and train staff on van and lift safety.
  • Ensure cleanliness of homes and that safety and sanitation standards are met.
  • Complete weekly grocery orders and ensure menus are available.
  • Ensure that staff are aware of any dietary requirements for individuals.
  • Train staff on the importance of meal preparation and mealtime practices.
  • Shop for individuals to replace clothing, shoes, and other items.
  • Manage CTF, petty cash and financial logs.
  • Submit receipts and requests as needed for purchases.
  • Ensure accurate and timely documentation through daily checks and audits, and address any deficiencies.
  • Perform home inspections and correct deficiencies.
  • Notify maintenance of any needed repairs.
  • Maintain relationships with families and other stakeholders.
  • Ensure completion of all evacuation drills and that all safety equipment and emergency supplies are available.
  • Ensure behavior tracking is being completed.
  • Submit reports weekly.
  • Manage medical appointments and schedule as needed.
  • Arrange for transport to appointments, medical emergencies, and activities.
  • Perform all other duties as assigned.
