About The Position

As a Community Liaison, you are the first impression of Tapestry Hospice for referral partners, patients, families, and the broader community. Your mission is to build strong, meaningful relationships that support access to compassionate end‑of‑life care. You will develop and strengthen referral relationships with physicians, hospitals, assisted living facilities, skilled nursing facilities, private practices, and other community health partners. You will manage a defined territory of accounts while continually identifying new opportunities for outreach. You will represent Tapestry with professionalism, warmth, and a genuine commitment to patient-centered care. You will promote our services through strategic marketing, education, and relationship-building activities. Responsibilities
