The Department of Health and Human Services Housing Division supports individuals with accessing and maintaining housing. We strive to provide Milwaukee County residents in need with a safety net, decent and safe affordable housing, and a connection to opportunities which improve their quality of life. We use available funding sources in the most equitable and efficient manner. Under direction of the Program manager the Community Intervention Specialist My Home, maintains a case load of homeless and formerly homeless participants with targeted disabilities of severe and persistent mental Illness and substance use disorders. This position works to maintain participants housing stability, process annual paperwork to determine rent amounts, accurately input participant information in Yardi and HMIS databases, conduct Housing Quality Standards (HQS) inspections, and act as a liaison between case management, landlords, and tenants. The Community Intervention Specialist will also advocate on behalf of program participants and provide case management services to 25 formerly homeless individuals.
