About The Position

Community Options, Inc., a national nonprofit agency providing services to individuals with disabilities in 11 states, is hiring a Community Integration Counselor in Butler, Pennsylvania. The Community Integration Counselor assists individuals with developing physical, intellectual, emotional, and social life skills. The role provides individualized services to help participants manage and overcome challenges associated with living in the community. Starting pay is $17.25 per hour.

Requirements

High school diploma or GED.
Valid driver’s license with a satisfactory driving record.
At least two years of relevant experience supporting individuals with intellectual and developmental disabilities.
Ability to establish a comfortable and supportive relationship with individuals receiving supports.
Successful completion of criminal background, central registry, child abuse registry, and drug testing checks is a condition of employment.

Bachelor’s degree preferred.

Responsibilities

Support and encourage individuals to increase their ability to be as independent as possible.
Meet with waiver participants regularly in their residence, in the community, or in an office setting.
Provide ongoing support for community integration efforts.
Document individual progress toward acquiring community integration.
Help build a support network between individuals and the community.
Administer medications and ensure required documentation is completed properly.
Coordinate leisure and volunteer opportunities.
Monitor individuals’ health and medical needs and immediately report concerns.
Drive individuals to medical appointments and activities in the community.
Complete program documentation and billable records accurately and on time.
Cooperate with licensing and department staff during inspections, inquiries, or investigations.
Perform additional tasks and responsibilities as assigned.
