About The Position

NorthKey Community Care is seeking a Community Integration Behavioral Health Specialist to work at various locations throughout Northern KY. Hours are generally Monday through Friday, 8:00 a.m. to 5:00 p.m., with flexibility to meet client and program needs. Some evening, weekend, and on-call responsibilities may be required. This position provides community-based assessment, therapy, rehabilitation, outreach, care coordination, and recovery support services to individuals with serious mental illness and co-occurring disorders. Services are delivered in homes, personal care homes, correctional settings, courts, hospitals, and other community locations to support recovery, community stability, and successful integration into the community.

Requirements

  • Master's degree in social work, Counseling, Psychology, Marriage and Family Therapy, or related behavioral health field from an accredited institution.
  • Licensure (CSW, LCSW, LPCA, LPCC, LMFTA, LMFT, or equivalent) or eligibility for licensure with clinical supervision.
  • Valid driver's license, acceptable driving record, and reliable transportation required.

Nice To Haves

  • Experience working with individuals with serious mental illness preferred.
  • Experience in community behavioral health, ACT, FACT, forensic services, AOT, residential services, or correctional settings strongly preferred.
  • Knowledge of evidence-based practices, recovery-oriented care, and trauma-informed treatment.
  • Strong communication, organization, documentation, and interdisciplinary collaboration skills.

Responsibilities

  • Conduct comprehensive behavioral health assessments and ongoing clinical evaluations.
  • Perform assessments of new clients promptly and complete updated assessments at least annually or upon significant changes in condition.
  • Diagnose individuals using the current edition of the Diagnostic and Statistical Manual.
  • Provide individual, family, and group therapy utilizing evidence-based practices.
  • Provide therapy, counseling, intervention, education, and skill-building services as identified in the client's treatment plan.
  • Develop treatment plans based upon client-identified goals, diagnoses, symptoms, and areas of need.
  • Utilize the most appropriate evidence-based practices to address client needs.
  • Provide clinically appropriate treatment based on assessments and treatment plans.
  • Address trauma and promote recovery for individuals with serious mental illness and serious emotional disturbance.
  • Provide psychoeducation and support to clients, family members, and caregivers.
  • Provide services and conduct self in a manner consistent with the professional Code of Conduct of NorthKey Community Care.
  • Deliver services in community settings including homes, personal care homes, hospitals, courts, correctional facilities, and other community locations.
  • Function as a member of a multidisciplinary Community Integration/ACT team.
  • Conduct assertive outreach and engagement activities to maintain participation in treatment.
  • Serve as a liaison between behavioral health services, community agencies, criminal justice partners, courts, probation, parole, detention facilities, and other stakeholders.
  • Provide treatment and monitoring services for individuals participating in Assisted Outpatient Treatment (AOT) under Tim's Law.
  • Coordinate services and support for individuals involved in the criminal justice system.
  • Provide behavioral health assessments, consultation, and therapeutic interventions within personal care homes and residential settings.
  • Assist clients in accessing community resources, healthcare, housing, employment, and social supports.
  • Participate in crisis intervention and stabilization efforts as needed.
  • Complete timely and accurate documentation regarding assessments, therapy, care coordination, and clinical interventions, and meet all documentation requirements as defined by NorthKey policies, Medicaid regulations, and applicable program standards.
  • Complete reports, including those for payers, court systems, and others, timely and appropriately. and ensure continuous reimbursable services for assigned clients.
  • Contribute to a culture of safety and quality by utilizing data to support decision-making, improve services, and ensure positive outcomes.
  • Implement and maintain the Behavioral Healthcare Program's philosophy, principles, and procedures in all aspects of service delivery.
  • Provide services and conduct oneself in a manner consistent with the NorthKey Community Care Code of Conduct.
  • Participate in planned supervision, case consultation conferences, multidisciplinary staffing, and team meetings.
  • Come prepared to meetings and keep clinical leadership informed of significant client and program issues.
  • Fully participate in staff development opportunities, trainings, and continuing education activities to maintain licensure, certification, and professional competencies.
  • Participate in on-call responsibilities as assigned.
  • Perform other duties as assigned.
