About The Position

This is an internal job posting for former Multnomah County employees on an active recall list. The Health Department is seeking a Work out of Class Community Information Specialist to join their clinic team. This critical role focuses on managing the complete patient referral process, supporting providers and patients. The position involves proactively managing and coordinating patient accounts, ensuring seamless information exchange, and building partnerships with external healthcare providers and insurance companies. This is a temporary backfill position expected to last no more than 6 months, with a hybrid telework schedule (one day in the clinic). Telework is contingent on meeting program KPIs.

Requirements

  • Education/ training equivalent to an associate's degree
  • One (1) to four (4) years of relevant experience that demonstrates the ability to perform the duties of the position.
  • Must pass a reference check.

Responsibilities

  • Determine reimbursement sources and navigate complex pre-authorization protocols.
  • Complete required documentation, processes, and track referrals.
  • Act as a liaison with Primary Care Access and Referral for self-pay referrals.
  • Coordinate with the OHP Eligibility Specialist to assess insurance coverage and identify appropriate specialty clinics.
  • Track and close referrals per policy.
  • Provide weekly updates to providers on high-risk referrals.
  • Follow up on outstanding reports.
  • Consult and advise providers on difficult referral situations.
  • Act as a liaison between client, parent/guardian, provider, specialty office, and insurance provider.
  • Provide assistance to the public regarding referral questions.
  • Collaborate with provider teams to support integrated and coordinated specialty care.
  • Research and problem-solve issues related to service access and reimbursement.
  • Provide clinical operations support (paper flow, faxing, certified mail, research, reports).
  • Create Reporting Workbench Reports for referral work queue, productivity, and volume.
  • Assist in maintaining electronic documents for distribution and publication.
  • Collaborate with Clinical Information Systems (CSI) to ensure patient record accuracy and identify EHR issues.
  • Assist in the review of Quality and Compliance related issues for primary care and dental referrals.
  • Assist in the review and assessment of project/program effectiveness and services provided through contract resources.
  • Maintain inventories of information materials.
  • Participate in clinic presentations and in-service training sessions.
  • Participate and lead meetings and training seminars related to patient education, provider onboarding, and specialty clinic updates.
