About The Position

The Community Health Worker will interview and evaluate applicants for services, formulate service plans and goals, and aid participants in implementing these plans. This role involves working with participants to overcome barriers and achieve goals, assisting them in advocating for themselves and moving towards self-sufficiency. The position also includes providing education on topics such as childbearing, reproductive health, child safety, parenting, and infant and child development. The Community Health Worker will act as a liaison and advocate for participants with referral sources concerning their progress in education, healthcare, and housing. Additionally, they will locate and connect participants to appropriate community resources and conduct street outreach to recruit participants from local businesses, schools, hospitals, and clinics. Creative outreach strategies, including mailings, home visits, baby showers, and door-to-door flyer distribution, will be used to motivate participants to engage with the CHW program services.

Requirements

  • High School Diploma or GED
  • 4 years of applicable experience and/or equivalent experience.

Nice To Haves

  • Bi-lingual Haitian Kreyol/English or Spanish/English is Preferred.
