About The Position

Under the direct supervision of the Patient Care Manager, the Community Health Worker (CHW) is responsible for helping patients and their families navigate and access community services, adopt healthy behaviors, improve their overall health and wellbeing. The CHW serves as a bridge between the health center, the community, social service and healthcare systems, providing education, support and advocacy. This position involves working closely with health care providers, community partners, patients to address social determinants and reduce barriers to access care. The CHW aims to prevent disease, disability and other chronic health conditions or their progression, while promoting overall physical and mental health. The services provided by the CHW are person-centered and patient driven, empowering individuals to advocate for themselves and receive personalized care. Services are provided through telephone, community outreach and office visits.

Requirements

• High school diploma or GED.
• Must possess or have the ability to obtain Community Health Worker (CHW) Certification.
• Possess valid driver’s license.

Nice To Haves

• Experience with social services, public health or mental health systems preferred.
• Lived experiences with mental illness, developmental disabilities, or substance use disorders are valued.
• Bilingual written and verbal skills are preferred.

Responsibilities

• Interview patients to assess needs, complete appropriate checklist, and confer with others to identify the nature and degree of concern.
• Facilitates referrals to health and social services and community resources.
• Research and record patients and community’s resources information; perform data entry as appropriate.
• Advocates for clients and serve as a liaison when appropriate
• Actively engage, re-engage, or ensure patient-led follow up in primary care; routing preventative care, adherence to treatment plans; and /or self-management of chronic conditions.
• Works with Patient-Centered Medical Home locations in Muskegon to identify and facilitate access to primary care and behavioral support services.
• Assist patients with making and keeping appointments and assist Patients find and access other relevant community resources.
• Assist patients with telehealth appointments and /or education on the use of telehealth technology.
• Connect patients to a medical translation/interpreter or transportation services as needed.
• Serves as a cultural liaison and assists medical providers with creating a plan of care as part of the healthcare team.
• Provide education on the following as needed with the oversight of a licensed provider: family violence, control of certain health conditions, chronic disease self-management, chronic pain management, sexual and reproductive health, and improvement in safety and environmental health of housing, reduction in the misuse of alcohol or drugs, stress management and reduction.
• Make necessary referrals and enroll individuals in Federal, State, local and private or nonprofit human services programs.
• Complete appropriate documentation within MFC’s EMR.
• Collect statistics and maintain records.
• Provide access to training and support to empower individuals to manage their own care and services to build individual and community resilience, central to the discussion and effectiveness of trauma informed approaches.
• All other duties as assigned.
