About The Position

Are you committed to directly serving your community and making a difference in the lives of those who need support by using your skills and strengths as a collaborative team member? As the Community Health Worker, we will count on you to provide Healthy Start Program services to at-risk families by getting involved in the community and sharing the resources and opportunities available to support their growing family journey. You will engage in outreach and recruitment efforts while enrolling families into the Healthy Start Program, team case management, screen health and safety questionnaires, review health information, and seek community referrals to assist at-risk families of South Phoenix and surrounding communities.

Requirements

  • Two years of experience working with pregnant women, infants, and/or toddlers in a social service program such as WIC, Parents as Teachers, Head Start, or another home visiting program, enrolling, conducting outreach, and/or case management in a health or social service setting
  • High School Diploma or GED
  • Related post-secondary education may substitute for the experience requirement on a year-for-year basis
  • Valid Arizona driver's license is required upon hire
  • CPR and First Aid (adult and infant) will be required within the first six months of employment
  • Must complete background check and fingerprint identification upon hire to successfully secure and maintain the Arizona Department of Public Safety Level 1 Fingerprint Clearance Card
  • Within one month of hire, all employees are required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test

Nice To Haves

  • Experience in perinatal healthcare
  • Bilingual skills in English and Spanish
  • Certification as a Public Health or Community Health Worker from an accredited college, or currently enrolled in a Public Health or Community Health Worker program, and eligible to receive certification within six months of hire

Responsibilities

  • Works outside of the office to meet with families, recruit and enroll eligible families into Healthy Start Program services, and assist in scheduling families with a case manager
  • Collaborates with participants, partner agencies, and providers in the community over the phone and in person to ensure that health care and social service needs are being met
  • Provides follow-up by telephone, mail, and email to ensure that needed services are received
  • Participates in outreach recruitment and education activities in the community by distributing brochures and informational materials, speaking one-on-one with residents, and giving presentations to raise awareness of services and issues regarding prenatal care, infant health, and ways to reduce infant mortality
  • Works outside of the office to recruit and enroll eligible families into Healthy Start services and secure additional community resources for potential referrals
  • Completes data entry of client documentation for follow-up, data reports, and monitoring of the Healthy Start project
  • Participates in quality assurance, customer satisfaction, and project planning by submitting participant cases for periodic review, attending regular case management meetings with the team, and presenting various cases to the team for discussion
  • Obtains feedback from participants on program services through surveys and makes recommendations to the management team on implementing new practices that can contribute to the success of the Healthy Start program
