About The Position

As a health division of the Confederated Tribes of the Goshute Reservation, Sacred Circle Healthcare preserves the Goshute heritage of protecting and caring for family and extends the tradition to all marginalized and underserved populations in our area. We refuse to let access, ethnicity, or socioeconomic status restrict someone from receiving complete healthcare. Sacred Circle Healthcare is seeking a full-time Community Health Worker for Ibapah, Utah. This role involves planning and organizing health/wellness events for the CTGR population, with a focus on elders and youth residing along the Wasatch Front. The position requires significant public interaction, coordination with tribal workers, and the provision of health and wellness education. The Community Wellness Specialist will be responsible for submitting written activity reports, administering satisfaction surveys, and traveling as needed.

Responsibilities

  • Plan and organize health/wellness events for the CTGR population 2-4 times a month, focusing on elders and youth along the Wasatch Front.
  • Coordinate events with tribal workers.
  • Provide health and wellness education presentations and activities to CTGR members.
  • Provide written activity reports to the Asst. Ex. Director.
  • Administer, collect, and record satisfaction surveys/feedback from community members.
  • Develop and implement wellness events for CTGR members (2-4x a month).
  • Work in collaboration with CTGR Goshute Clinic staff, CTGR Social Services, CTGR Education, and SCHC providers.
  • Work in collaboration with the SCHC marketing team and SCHC staff.
  • Conduct satisfaction surveys and compile a monthly report from the data.
  • Prepare written reports after each event and submit them to the Strategy Officer.
  • Maintain scheduling of events.
  • Assist with requesting patient healthcare supplies.
  • Assist with Medicaid applications and follow-up on applications.
  • Read, understand, and adhere to all Sacred Circle Health Care Policies and Procedures.
  • Practice and adhere to the Code of Conduct and Mission and Values Statements.
  • Perform other duties as assigned.
