About The Position

IMPaCT Care is seeking 10 energetic Community Health Workers (CHWs) for a new partnership with Highmark Health in Pittsburgh, PA. While the role supports Highmark Health's community health initiatives, you will be directly employed by IMPaCT Care. This is a field-based position where you will spend the majority of your time in the community, including clients' homes, hospitals, clinics, and community organizations. You will also report regularly to Highmark Health locations for team meetings, training, and care huddles. The role requires building trust through consistent presence and culturally aligned, empathetic communication, identifying and addressing barriers to care such as food insecurity, transportation, and housing, and connecting patients to care teams and community resources, with follow-up to ensure needs are met. Documentation of client interactions, needs, assessments, and outcomes in the EPIC EMR system, completion of SDOH screenings, and participation in integrated care huddles are also key responsibilities. The position emphasizes using feedback and performance data for growth and refining approaches based on client outcomes.

Requirements

  • High school diploma or equivalent
  • 1–3 years in community-based outreach, health education, or related work
  • Valid driver's license
  • Must own a vehicle
  • Must have car insurance
  • Ability to pass background check and pre-employment screening
  • Flexibility to cover multiple locations across the area
  • Applicants must be U.S. citizens.

Nice To Haves

  • Bilingual Spanish preferred
  • One year of CHW experience
  • Experience with an EMR system such as Epic
  • Familiarity with Medicaid, D-SNP, waiver programs, and recertification cycles
  • Experience with SDOH screening tools (e.g., Hunger Vital Sign, USDA Household Food Security) or case management platforms
  • Knowledge of HEDIS (Healthcare Effectiveness Data and Information Set) metrics or risk stratification concepts
  • Strong ties to community organizations: faith institutions, shelters, food banks, or mutual aid networks

Responsibilities

  • Build trust through consistent presence and culturally aligned, empathetic communication

  • Connect patients to care teams and community resources; follow up to ensure needs are met
  • Document all client interactions, needs, assessments, and outcomes in the EPIC EMR system

  • Participate in integrated care huddles and collaborate with clinical teams
  • Use feedback and performance data as tools for growth, actively refining your approach based on what the metrics show about client outcomes
