About The Position

Mass General Brigham is a not-for-profit organization dedicated to patient care, research, teaching, and community service, striving to provide exceptional care. Affiliated Pediatric Practices (APP) is a network of independently owned Massachusetts-based community pediatric practices with over 30 years of experience. APP aims to offer excellence in healthcare to pediatric patients and families by combining high-quality care with outstanding efforts to provide exceptional patient experiences. The network includes more than 100 physicians, 45 nurse practitioners, and 15 behavioral health providers across 32 offices in Massachusetts. This unique collaboration, along with a partnership with Mass General Brigham, enhances their ability to exceed national pediatric healthcare standards. This full-time, 40-hour Community Health Worker (CHW) position is a hybrid role with a flexible schedule, ideally Monday – Friday, 8 am – 5 pm, involving 4 days on-site and 1 day remote. The CHW will primarily support from the Dedham office 2 days per week and will also be embedded 1-2 days per week at other APP practices within a reasonable distance to provide in-person support. The CHW will work closely with patients, families, and community resource agencies to promote and maintain good health, providing education, support, and outreach services to help community members achieve their health goals.

Requirements

  • High School Diploma or Equivalent
  • Strong communication and interpersonal skills, with the ability to interact effectively with diverse populations.
  • Ability to work independently and as part of a team.
  • Basic computer skills, including Microsoft Office and database management.

Nice To Haves

  • Associates Degree or Bachelor's Degree Related Field of Study preferred
  • Experience in community outreach, health education, doctor's office, or related field (0-1 year preferred)
  • Bilingual skills preferred; Languages in order of greatest demand: Cape Verdean Creole, Brazilian Portuguese, Spanish, Haitian Creole
  • Demonstrated ability to work effectively and provide advocacy for racially and ethnically diverse populations and communities preferred.

Responsibilities

  • Work closely with patients, families, and community resource agencies to promote and maintain good health.
  • Provide education, support, and outreach services to help community members achieve their health goals.
  • Address areas of need most commonly including patient access to resources such as food, housing, childcare, and financial support.
  • Address behavioral health/substance use/medication management, transportation, job skills training, and healthcare system navigation.
  • Deliver preventive care and health education to the community under the direction of a healthcare professional.
