The CHW will be instrumental in building the Enhanced Care Management program for Shasta Cascade Health Centers. This role will be front facing with patients and families in order to determine if patient(s) qualify for the ECM program and then support in accessing appropriate programs for their needs. This position requires a majority of patient facing time in the community, as well as time spent collaborating with the various programs in the community that may serve as resources for patients, such as community resources centers, food banks, mental health services, public health services and more.

Requirements

  • High School Diploma or GED required
  • Two years working in social work, community outreach, or customer service
  • Must successfully complete an approved Community Health Worker program within 12 months of hire
  • Basic Life Support (BLS) certification, or ability to become certified.
  • Must be self-motivated and outgoing
  • Operate with the highest of ethical standards
  • Must be able to relate and communicate positively, effectively, and professionally with others
  • Must be able to think critically and problem solve
  • Manage multiple assignments effectively, be organized, and able to work independently with minimal supervision.
  • Remain calm in emergency situations
  • Be open to collaboration
  • Knowledge of the needs and barriers people with disabilities, seniors and financially disadvantaged people within the community face
  • Must have an understanding of the available resources within the community
  • Ability to communicate clearly and at a professional level both verbally and in writing
  • Ability to communicate effectively in person, by phone and by email.
  • Ability to visually interpret printed materials, memos, and other needed paperwork.
  • Ability to maneuver immobile patients and to lift and carry children weighing up to 40 pounds.
  • Ability to lift or move objects weighing up to 25 pounds.
  • Have sufficient stamina to stand and walk for full work shifts.

Responsibilities

  • Assisting patients and their families navigate and access community services, health services, and adopt healthy behaviors.
  • Working collaboratively with patients and their families, community resources, county programs and clinical staff as appropriate.
  • Identifying barriers to care and sharing that information with Leadership for quality improvement.
  • Maintaining confidentiality.
  • Supporting public health efforts to educate the community on infection diseases.
  • Delivering information about health and wellness in ways that the community can easily understand.
  • Establishing positive and supportive relationships with patients and those who work within the community/county programs to foster positive experiences and outcomes.
  • Motivating patients to be active and engaged in their health.
  • Objectively reporting data and identifying targeted areas of program activity.
  • Documenting activities as required within the provided systems.
  • Other duties as assigned.

  • Maintaining operations by following the Clinic’s policies and procedures, and reporting perceived need/s for changes.
  • Providing the highest level of service to patients.
  • Maintaining confidentiality by following all applicable HIPAA regulations.
  • Respecting Clinic equipment used to perform job tasks.
  • Determining if patient(s) qualify for the ECM program and supporting them in accessing appropriate programs for their needs.
