Engages a population of socially vulnerable patients in their care, assists them through the process of working towards better health by providing support, encouragement, and education in person, by telephone and via electronic means. Builds relationships with respect to diversity, using active listening, casual counseling to strengthen the patient’s health, social and personal goals. The CHW will operationalize the care coordinator’s care plan by providing on-going support to the patients, educate them on self-management of their health conditions, proper use of the emergency room, help patients to overcome barriers to obtaining medical care/social, access community resources, etc. Acts as a patient advocate and liaison between the patient/family and community service agencies (i.e., schools, DHS, hospital, etc.).
