The Community Health Worker (CHW) serves as a vital link between healthcare systems and vulnerable populations, supporting members through direct engagement, education, and coordination of services that improve overall health and stability. Pacific Health Group is committed to improving health outcomes through community-based care, addressing the full spectrum of social determinants of health. Our teams work directly in the field to support underserved populations and ensure members receive the care, resources, and advocacy they need to thrive. This role involves spending approximately 65% of your time in the field (homes, community sites, events) to conduct outreach, assessments, and direct member engagement. You will travel within the assigned county to meet members and community partners, support health education, care navigation, and resource coordination, and facilitate group sessions and community-based activities. Documentation of services and interactions will be done in real-time. This role requires strong time management, independence, and comfort working in diverse, community-based environments.
