About The Position

This position is responsible for outreach to Veterans and individuals in the community that are in need of rapid re-housing and homeless prevention services. It involves referring persons served in need of housing services to available resources and conducting initial assessments of housing support services, including strength-based needs assessments.

Requirements

High school diploma required.
Must self-identify as having a disability and receive or currently receiving services.
Demonstrated basic writing skills.
Demonstrated skills necessary to engage a diverse group of individuals.
Demonstrated ability to work in a team environment.
Current driver’s license issued by state of residence.
Current Vehicle Inspection decal.
Current vehicle insurance.
Current vehicle registration in state of residence.
Satisfactory Motor Vehicle Report.
Must be able to pass a criminal history investigation.

Nice To Haves

Community Health workers (CHW) preferred certification.
Veterans are preferred for this position.

Responsibilities

Conduct outreach services on Monday, Wednesday, Thursday evenings, and Saturday and Sunday mornings.
Connect individuals with benefits and supports of the case management team and intake/data specialist.
Communicate effectively with the Outreach Supervisor regarding community needs and coverage areas.
Conduct outreach in the 16 parishes covered by VOASELA via landlord communication and connections to regional resources and homeless shelters.
Verify homelessness of veterans and individuals in the community requiring services.
Provide support and direct assistance to tenants based on an individualized housing support plan and engagement of resources in the 16 region parishes.
Assist with transportation, communication, and self-advocacy as needed for Veterans and individuals.
Assist other team members as needed.
Advocate for veterans’ services in problem-solving conflicts and advocate for individuals needing VOASELA referrals and resource engagement.
Attend Stand-Down events and other community organized events.
Connect with local VA offices and GPD sites for assistance, as well as local CoC, shelters, street outreach, and housing facilities.
Visit the Community Resource Referral Center as scheduled by the Outreach Supervisor.
Perform other duties as assigned.
