About The Position

The Community Health Worker (CHW) provides support for Head Start families enrolled in Tier 1 and Tier 2 programs. This person will work in collaboration with staff and health professionals to support families in meeting the Head Start health requirements. The CHW will provide health and safety education and resources. The CHW will assist families in navigating the health care system to improve health outcomes for children.

Responsibilities

  • Maintain a thorough knowledge and understanding of Head Start Health Requirements and MARC Head Start health policies and procedures.
  • Support families in the completion of health goals, including Individualized Health Care plans, scheduling medical and dental appointments, and activating or re-activating Medicaid insurance.
  • Provide site-specific support and resources for Tier 1 and Tier 2 programs relevant to the needs of the site, the children and the families served by the program.
  • Document all interactions, supports, and follow-up with Head Start families in the ChildPlus data system.
  • Assist the contract Doula(s) to complete health screenings and other on-site health services, which include screenings during the 2-week post-partum visit.
  • Provide health and safety education to families, including car seat safety education and installation.
  • Prepare ChildPlus reports and monitor assigned sites for health screenings communicating needs and supports needed.
  • Participate in Health team meetings to identify barriers, challenges, and solutions in completing health requirements within the specified Head Start timeframe.
  • Support the Head Start Health Manager and Health Coordinators with internal and external health initiatives.
