The Community Health Worker (CHW) helps patients access and navigate HOPE Family Health services and community resources. This position is assigned to Behavioral Health Telephone Support, assisting patients by phone with scheduling, follow-up, and coordination of behavioral health services. The CHW communicates with patients and care teams, identifies barriers to care, coordinates follow-up, and accurately documents services. Duties depend on the assignment, training, credentials, and HOPE policies; individual CHWs are not expected to perform every duty listed in the position description. The CHW builds trusting relationships with patients and families, communicates in a culturally responsive manner, supports patient choice and self-advocacy, and connects patients with care teams and community resources. The role may include telephone support, referral coordination, language access, records and care coordination, and community engagement. Clinical triage, assessment, diagnosis, treatment, or clinical advice may be performed only by staff whose credentials, role, training, and HOPE authorization permit it. The position primarily takes place in a professional office or clinical environment, with occasional travel between HOPE sites. Off-site work is not required, but some tasks may be completed remotely with prior Team Leader approval and an appropriate confidential workspace. Occasional extended hours may be required based on organizational needs.
