About The Position

Requirements

  • H.S. Diploma or equivalent is required.
  • Six (6) months customer service experience either face-to-face or by phone is required
  • BCLS – Basic Cardiac Life Support is required within 6 months of hire.

Nice To Haves

  • Successful completion of a Community Health Worker formal training program such as from a community college or other education institution is preferred.
  • bi-lingual a plus.
  • Six (6) months of public health experience preferred
  • Community health worker certification with the state of Texas is preferred.
  • Bilingual Preferred

Responsibilities

  • Coordinate Community Health Improvement Activities
  • Participate in program development, planning and evaluation.
  • Develop and effectively manage program timelines to ensure timely completion of program deliverables.
  • Monitor program activities and reporting to ensure accuracy in outcome measures.
  • Facilitate small- and large-group education.
  • Establish and manage relationships with community members, organizations and coalitions.
  • Identify strategic partnership opportunities and promote relationship-building.
  • Disseminate programmatic and marketing materials.
  • Develop presentations and other materials for use during program-related events.
  • Serve as programmatic liaison to internal and external stakeholders.
  • Build and maintain collaborative relationships with FQHC and other low-cost clinics to support care coordination
  • Responsible for establishing trusting relationships with patients and families.
  • Navigates patients and families through medical and non-medical support services.
  • Conducts eligibility, enrollment and follow up for referred medical or non medical support.
  • Helps patients set personal care plans and attend appointments.
  • Works closely with medical care team to ensure coordinated care.
  • Service as liaison to internal and external resources through TH network.
  • Provides cross coverage support across entities and assisting fellow CHWs to ensure continuity of care.
