About The Position

The Community Health Worker plays a key role in connecting patients and families to the care and resources they need. This position focuses on community outreach, promoting wellness exams and screenings, reducing barriers to care, and supporting underserved and high-risk populations across NMHSI’s seven-county service area.

  • Making medical and dental appointments
  • Tracking outreach activities, patient contacts, and screening outcomes
  • Maintaining accurate and complete documentation within the electronic health record
  • Performing basic health checks such as blood pressure or glucometer readings
  • Building connections with community partners and providing resource referrals

Responsibilities

  • Support cancer screening coordination, including chart reviews, referrals, scheduling, and follow-up
  • Assist patients in accessing programs such as BCCCP and Quit Line
  • Educate community members about NMHSI services and the importance of preventive screenings
  • Conduct outreach at community events, farms, and local organizations (approximately 50% of time in the field)
  • Make medical and dental appointments for individuals identified through outreach
  • Track outreach activities, patient contacts, and screening outcomes
  • Maintain accurate and complete documentation within the electronic health record
  • Perform basic health checks such as blood pressure or glucometer readings at outreach events
  • Build connections with community partners and provide resource referrals
