The Community Health Worker is a member of the community who applies their unique understanding of the experience, language, and/or culture to communicate a comprehensive health education program to diverse members of the community specifically within educational partnership with patients and families. The Community Health Worker will be responsible for assisting patients with a variety of care coordination activities. Activities will vary based on patient need, but basic functions will include helping patients navigate the healthcare system, understand and be connected to CentraCare to eliminate barriers to care. The Community Health Worker will carry out these responsibilities in both a clinic and a limited community setting. In this position you will be the bridge to al communities, focused on clinic. Schedule: Full-time | 80 hours every two weeks Monday-Friday 8am-5pm Occasional community events on evenings or weekends Pay and Benefits: Starting pay begins at $19.91 per hour; exact wage determined by years of related experience Pay range: $19.91 – $29.87 per hour Full-time benefits: Medical, dental, PTO, retirement, employee discounts and more! Education: Associate Degree in health promotion, community health, licensed practical nurse or certified medical assistant program. At least two years of experience working in healthcare with community involvement in lieu of degree.
