About The Position

Sentara Health is seeking a Community Health Worker to support community outreach, resource coordination, and care navigation for individuals in the community. This role works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illnesses. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management. This role works with a variety of grant funded programs to bridge the gap between organizations, including health care delivery systems and community health services to: increase culturally relevant knowledge about health and wellbeing; improve underserved populations’ access to services; connecting patients to community resources; and ultimately improving health outcomes.

Requirements

  • High school diploma or equivalent
  • Valid driver's license

Nice To Haves

  • Knowledge of medical terminology
  • Community Health Worker (CHW) certification
  • Experience working in a community-based setting
  • Experience identifying and coordinating community resources
  • Epic experience
  • Spanish-speaking ability
