About The Position

The Community Health Worker (CHW) will serve as a trusted liaison for patients who may require follow up coordinated support beyond the medical care provided. This role focuses on improving access to care, reducing health disparities, and promoting overall well-being through outreach, education, and resource navigation. The CHW will work closely with community partners and human service agencies to connect patients with appropriate services and support.

Requirements

  • Bachelor’s in healthcare related field.
  • Two (2) to Three ( 3) years of experience in healthcare related field.
  • Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
  • Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
  • PA Community Health Worker Certification preferred/required.
  • Act 34-PA Criminal Record Check from the PA State Police system

Nice To Haves

  • Experience in case management or care coordination.
  • Familiarity with health equity principles and trauma-informed care.
  • Bachelor’s Degree in social work
  • Three (3) to Five (5) years’ experience in health and human services
  • Experience in Healthcare setting.

Responsibilities

  • Patient Advocacy, provide ongoing support through phone calls and visits to ensure patients adhere to treatment plans.
  • identify patient barriers to care and development of plan to address barriers.
  • Provide follow up via phone and in person visits for coordination of care.
  • Explaining healthcare information in a culturally sensitive, easy to understand manner
  • Care coordination, connecting patients to healthcare providers and human services resources.
  • Maintain accurate documentation of patient interactions and referrals.
  • Collaborate with healthcare providers, social workers, and agency staff to coordinate care.
  • Participate in community events and health promotion initiatives by conducting outreach and engagement activities in community settings.
  • Other duties as assigned.
