About The Position

Sentara Rockingham Memorial is seeking a Community Health Worker- Outreach for the Hand-in-Hand Virginia Resource Mothers Program - Home Visiting program, which supports newly pregnant and parenting teens. This role collaborates with the healthcare team to assist diverse populations by ensuring access to vital health services, equipment, and resources that promote overall health. The Community Health Worker will provide education on important health topics, encourage behavior change, screen for risk factors, and work with community organizations to implement effective outreach and programming. This position will also help individuals navigate public benefits, connect with support services, and foster positive health outcomes through coaching, education, and early intervention. The role engages with various grant-funded programs to bridge the gap between healthcare delivery systems and community health services, aiming to increase culturally relevant health knowledge, improve access to services for underserved populations, connect patients with community resources, and ultimately enhance health outcomes.

Requirements

  • Bilingual in English and Spanish is preferred.
  • Experience working in a community-based setting preferred.
