We are a private, not-for-profit organization that coordinates public health grants for a 26-county region in central PA, seeking a Community Health Worker (CHW). This is a full-time hybrid position located within our Camp Hill location, balanced with opportunities for work-from-home days. The Community Health Worker (CHW) will provide housing support services and other case management duties as assigned to clients with lower incomes in their communities. CHW will work collaboratively with all applicable internal and external stakeholders' staff and partners to identify needs and connect clients to housing, healthcare and social services.

Requirements

• High School Diploma or GED (General Education Diploma) is required
• CHW Certification will be required; training will be provided upon hire
• Strong verbal and written communication skills
• Time management and prioritization skills
• Computer literacy including Microsoft 365
• Ability to work with cross-disciplinary teams of diverse individuals, including those with lived experience
• Reliable transportation and a valid PA driver's license are required for travel within the specified area
• PA State Police Criminal Record Check (PATCH)
• FBI Criminal Record Check, from fingerprinting
• PA Child Abuse History Clearance (PACA)
• Up-to-date vaccinated status (influenza and COVID-19, but could include others)

• Prior CHW experience or related experience is a plus
